• Review your provider notes, prescriptions, medications, and lab results through May of 2024.

Activate your Phoenix Health portal today!

To access your Phoenix Health patient portal:

1. Visit the ���ɱ�������ٱ�.
2. Select 51������ from the drop-down menu and click “Remember my selection permanently” before clicking “Next.”
3. Log in with your Elon username and password.
